Takeshi Hongo, a motorcyclist, is abducted by the villainous Sustainable Happiness Organization with Computational Knowledge Embedded Remodeling (SHOCKER), which turns humans into synthetic animal-human hybrids called Augments. Hongo becomes Grasshopper-Aug 01, but before he can be brainwashed, former SHOCKER member Ruriko Midorikawa rescues him. Spider-Aug, one of SHOCKER’s executive agents, goes after them and is temporarily successful in capturing Ruriko before Hongo rescues him using his augmented suit.

They go to a safe house and wait for Ruriko’s dad, Dr. Hiroshi Midorikawa, to come get them. He explains that Hongo is his masterwork Augment to defeat the organization, and that he chose him because Hongo yearned for power after his police officer father was killed by a suspect he was trying to appease. He also explains that Hongo’s ability to tap into prana, the life source, grants him superhuman strength. Spider-Aug finds them and murders Dr. Midorikawa before trying to capture Ruriko again. In light of recent events, Hongo has renamed himself “Kamen Rider” and pledged to use his abilities for good. Despite his mistrust, Ruriko eventually comes to rely on Hongo’s skills.

Later, two government agents who remain nameless visit Hongo and Ruriko and persuade them to join forces in order to destroy SHOCKER in exchange for safety and information. According to Ruriko, SHOCKER is a secret group founded by a billionaire who wanted to bring peace and prosperity to the world. Nevertheless, the billionaire took his own life, leaving SHOCKER in the hands of an AI that misunderstood its creators’ intentions and came to believe that slavery was the key to saving humanity. On the government’s request, Hongo and Ruriko fight against a swarm of Augs, including Ruriko’s one and only friend Hiromi, who has been transformed into the Wasp-Aug. One of the nameless agents shoots Wasp-Aug with the Scorpion-venom Aug’s after Hongo refuses to kill her. Ichiro, Ruriko’s brother, eventually becomes active as the Butterfly-Aug, and she is unable to put the finishing touches on a code to control him. Ichiro murders many government operatives by damning their spirits to the Habitat Realm, a place of torment beyond the realms of the living. After hearing Ichiro’s fury over the random murder of Ruriko’s mother, Ruriko admits that he plans to exact the same revenge on humanity.

Ichiro is confronted by Hongo and Ruriko, but they are defeated with the help of SHOCKER newbie Hayato Ichimonji, transformed into Grasshopper-Aug 02. Ruriko and Hongo make it out alive, but Hongo gets injured by Ichimonji on the way. Nonetheless, Ruriko is able to free Ichimonji from SHOCKER’s influence. Ichimonji quickly dispatches the K.K.-Aug after it has slain Ruriko. Hongo, now fully recovered, reads Ruriko’s final will and testament through his helmet, in which she admits she lost to Ichiro because his prana was stronger, transfers a code into Hongo’s helmet that will render Ichiro harmless, and says that her time spent with Hongo was the happiest of her life. Hongo confronts Ichiro after he asks the two unidentified agents for a favor after he mourns her death and fails to convince Ichimonji to join him against SHOCKER.

As Hongo is ambushed by a group of Grasshopper-Augs, Ichimonji, who calls himself “Kamen Rider No. 2,” comes to the rescue and together they work to fight the Augs. Ichiro turns into Kamen Rider No. 0 to face them, and despite Hongo and Ichimonji damaging his source of prana, he still manages to show his strength. Together, they succeed in exchanging Ichiro’s helmet for Hongo’s, allowing him to communicate with Ruriko’s spirit and ultimately bring about their reconciliation. Ichiro and Hongo both succumb to death from prana exhaustion. Later, the two government operatives disclose that they have safely stored Ruriko’s soul and that Hongo wants Ichimonji to continue the name of “Kamen Rider” and destroy SHOCKER by giving him his helmet. Reluctantly, Ichimonji agrees to cooperate with the agents, who introduce themselves as Tachibana and Taki. Together, Ichimonji and Hongo, who has transferred his soul into the helmet, are now known as “Kamen Rider No. 2+1,” and they continue their journey into a brighter future.
