John Wick and the Bowery King are in hiding in New York City while they plan their revenge on the High Table. In Morocco, he murders the Elder, the proverbial “one who sits above the Table.” Marquis Vincent Bisset de Gramont, a member of the High Table, responds by calling Winston, the manager of the New York Continental hotel, and Charon, the concierge, and telling them that the High Table has given him unlimited resources to kill John and berating Winston for his failure to do so in the past. The Marquis removes Winston’s administrative responsibilities, labels him “excommunicado,” orders the destruction of the New York Continental, and has Charon put to death as punishment. Caine is a blind, retired assassin from the High Table, and the Marquis recruits him to assassinate his old buddy John, threatening to kill Caine’s daughter if he refuses.
The Osaka Continental, operated by John’s friend Shimazu Koji, becomes John’s safe haven. Chidi, the Marquis’s second-in-command, shows up at the hotel accompanied by assassins from the High Table and Caine. Akira, the concierge and daughter of Koji, leads the evacuation of the hotel just in time to avoid the chaos that will ensue when the High Table “deconsecrates” the building. John fights off waves of armored assassins until he finally faces Caine in a final encounter. A bounty hunter known only as “Mr. Nobody” intervenes during their struggle to let John escape after deciding the reward for his death is too low. Injured Koji keeps attacking Caine on John’s behalf, and Caine finally decides to murder him while sparing Akira.
When John gets back to New York, he finds a vengeful Winston waiting for him at the grave of Charon. Invoking an ancient High Table custom, Winston advises that John challenge the Marquis to a duel. John can only request a battle on behalf of a crime family, but if he wins, he will be released from his responsibilities to the High Table. John tries to reestablish contact with the Ruska Roma criminal organization by visiting their headquarters in Berlin. John’s adoptive sister Katia requires him to avenge the death of her father by eliminating Killa Harkan, a member of the High Table who was responsible for the crime. John’s nightclub is ambushed by Killa, but with the help of Caine and Mr. Nobody, he is able to murder Killa and reclaim his title.
As John’s “second,” Winston informs the Marquis of John’s challenge and asks that, should John be victorious, the New York Continental be rebuilt with full funding from the High Table and that he be reinstated as the manager. At a meeting in Paris presided over by the Harbinger, the Table’s envoy, John and the Marquis settle on the terms of the duel: pistols at sunrise the following day at Sacré-Coeur. The Marquis appoints a hesitant Caine as his successor, and the Harbinger announces that John and Winston will be executed if they don’t show up. John receives a weapon and a new ballistic suit from the Bowery King, who travels to Paris to see him.
The Marquis has placed a $26 million reward on John in the hopes that he would be delayed in his journey to the combat. John faces a swarm of killers on his trip to Sacré-Cur and has to fight them off. Along the way, he meets Mr. Nobody, who raises the demand on his head to $40 million. John stops Chidi from murdering Mr. Nobody’s dog during their fight, prompting Mr. Nobody to give up his pursuit of John. Caine and Mr. Nobody help John fight off many assassins on the Rue Foyatier, and they make it to the summit in time for the duel. Mr. Nobody kills Chidi. After two rounds of dueling in which both John and Caine suffered wounds, the third round was called off when Caine seriously injured John. The Marquis enthusiastically offers to switch places with Caine in exchange for the opportunity to deliver the fatal blow. John shoots the Marquis dead after being reprimanded by Winston, who reveals that he has not yet used his third bullet.
Caine and John are released from the High Table and Winston is brought back thanks to the Harbinger. Before passing out from his wounds, John looks back on his life and marriage. At John and Helen’s gravesite in New York, Winston and the Bowery King said their final goodbyes.
